Give enough detail for us to locate the material on our service, for example the channel name, title, date and time, or a URL on elitetv4kus.com.
Provide your contact details
Include your full name, organization (if any), postal address, telephone number and email address so we can contact you about the notice.
Sign the required statements
Include a statement that you believe in good faith the use is not authorized, a statement under penalty of perjury that the notice is accurate and that you are authorized to act, and your physical or electronic signature.

4. Counter-Notices
If you believe material was removed by mistake or misidentification, you may send a counter-notice that includes your contact details, identification of the removed material, a statement under penalty of perjury that you believe it was removed in error, consent to the jurisdiction of the appropriate court, and your signature.
Accounts of users who repeatedly infringe copyright will be terminated in appropriate circumstances.
